Make Your Mark with a Ball and Sport Relief stickers

Download these stickers (Word doc) and print them off using Avery L7163 labels. Stick them on your clothes, balloons or bags and let everyone know you're raising money for Sport Relief!

Make Your Mark consent form

If you are under 16 years of age you must submit a signed consent form (Word doc). This needs to be signed by a parent, guardian or teacher and must be returned by fax (020 7497 4040), email (ball@makeyourmark.org.uk) or post to Lauren Maleh at Make Your Mark (6 Mercer Street, London, WC2H 9QA) by the closing date (Sunday 16 March). This is compulsory and if you are under 16 and fail to send a signed consent form your entry will be disqualified.
